Frank P. Pulcini - Vice President
Frank is the founding partner of the Fort Lauderdale and Miami branch of IOA and helps oversee the growth of Insurance Office of America in the Southeast Florida Region. He has in-depth knowledge of the Insurance Industry with over 26 years of risk management experience including product-related expert witness work and litigation management for an international supply firm. Frank has worked for two of the top five Broker / Agencies in the world and continues to represent IOA and his clients in the areas of traditional Risk Transfer Programs as well as Captives and Large Deductible Programs.
Frank oversees all account management as well as Program Structure, Placement of Coverage, Claims Management and Loss Control Services.
Frank is a Shareholder and Advisory Council Partner at Insurance Office of America.
Javi Fernandez
With more than 12 years of experience providing Insurance and Risk Management Services to small to mid-sized companies, Javier’s strengths are his strong technical and negotiation skills with Commercial & Multi-Family Property, General, Product, Professional & Management Liability, Worker’s Compensation, Medical Malpractice and Contractual Requirements. More than his experience, however, he prides himself on always representing the best interests of his corporate clients.
Throughout his career, Javier has represented clients in sectors such as Construction & Development, Hospitality, Logistics & Transportation, Marine, Healthcare, Commercial & Multi-Family, Life Sciences, Manufacturing, Legal, Accounting & Finance, and Education & Vocational Training. His specialties are analyzing, implementing, educating and executing.
Javier has considerable operational experience having served as Principal of his own firm for close to ten years. His company leadership, strong entrepreneurial spirit and ability to adapt to changing times provides for great synergy between himself and other business leaders. “Great business is conducted based on greater relationships”.

Andrew Giambarba, MLIS
Andrew is an Insurance and Risk Management Specialist focused on providing commercial and professional liability coverage solutions for middle-market clients anywhere in the United States. Specializing in Errors & Omissions, Directors & Officers Liability for Private or Public Companies, Employment Practices Liability Insurance (EPLI) and all other Management Liability products including Cyber/Tech/Privacy Liability as well as Product Liability for both English and Spanish speaking clients.
A graduate of Tulane University, Andrew is a Board Member of the Independent Insurance Agents of South Florida (IIASF), the co-Chairman of the InVEST Program, a member of the Professional Liability Underwriting Society (PLUS) and holds the IRMI designation of “Management Liability Insurance Specialist” (MLIS). Fluent in Spanish and English, Andrew strives to develop long-lasting business relationships through exceptional service and competitive marketing.
Carolina Barasoain Fernandez
Carolina Fernandez is an experienced Employee Benefits Broker who provides strategic guidance and hands-on assistance across all areas of Employee Benefits. Her strengths include workplace practices, benefit administration, strategic planning and alignment, financial modeling, training and overall management. Her experience includes managing major national accounts as well as extensive experience in Employee benefits and HR management.
Her formal education includes a Bachelor of Science in Public Relations and a minor in Business Administration from the University of Florida. She is an active member of the Coral Gables Chamber of Commerce and the Miami Beach Chamber of Commerce. She also serves as a young ambassador for Miami Children’s Hospital.
Robert “Bob” Mihm, MBA
As the former owner of a major South Florida business, Bob understands firsthand how the cost of insurance and proactive risk management can impact the bottom line. IOA’s unique Risk Services professionals help Bob’s clients lessen the impact of claims and numerous aggravated inequities awards.
Bob’s areas of specialty include the construction/development sector, environmental and waste industries, marine related (product manufacturing and marinas) and providing clients with expert analysis of outsourcing solutions.
